Sepsis

-Definition:
     -SIRS plus infection

-Severe Sepsis:
     -Sepsis plus at least 1 of the following:
          1) Mottled skin
          2) Capillary refilling >= 3 sec
          3) UOP < 0.5 mL/kg x1 hour or requiring CRRT
          4) Lactate > 2 mmol/L
          5) Abrupt mental status changes
          6) Abnormal EEG
          7) Plat's < 100,000
          8) DIC
          9) Acute lung injury (ALI) or ARDS
          10) Cardiac dysfunction --> by EKG or direct measurement of cardiac index (CI)

-Septic Shock:
     -Severe sepsis plus 1 or both of the following:
          1) MAP < 60 mmHg (or < 80 mmHg if baseline HTN) despite adequate IVF resuscitation (PCWP 12-20)
          2) Maintaining MAP > 60 mmHg (or < 80 mmHg if baseline HTN) requires pressors
